Leo, a 1.5 year old Akita, presented to Blue pearl on May 28, 2024 after jumping out of a vehicle, in April going 50 mph and injuring his LF limb. During his consultation, he was diagnosed with left front radial nerve injury, ulcerative lesions on his LF digit pads, and an erosive lesion over his left front dorsal metacarpophalangeal joint #2. In July 2024, Leo was seen at Pet Wellness Center in Ft. Myers for a consultation in which Rehabilitation was recommended. August 30, 2024, Leo was seen at our clinic, Burnt Store Animal Hospital, for a rehab consultation.
Upon arriving for consultation, you could notice the severe limping and neurological deficits in the LF limb. P was constantly knuckling, rolling the paw over onto the metacarpal, and hopping on his “good side”. During the consultation, Dr. Jeremy spoke with O on diagnosis, possible secondary injuries, future results, supplements, and diet. A Stance Analyzer, by Companion, was performed which showed the P only placing 6% of his body weight on the effected limb. The main goal is to restore the patient to normal function. The treatment plan was to use the underwater treadmill, multi radiance lasers, and the Sports Innovations PEMF mat, to allow weight bearing onto limb, increase blood flow, reduce inflammation and infection (blue light), support healing of the paw, and generalized wellness. The O was instructed to do controlled leash walks and Range of Motion on shoulder, elbow, and metacarpals.
The plan for the first package was to use 10 sessions 2 times a week. Each week the P was tested a little more, whether it be time and incline in the UWTM, using no-knuckle training wraps, and using the rehab equipment to challenge the P more on balance and proprioception. At the end of each session the P was placed onto the PEMF mat which was set at Regenerate for 20 mins at 75% of power. This setting was used for the first 2 weeks of rehab. The following weeks after, the P was using the mat at 100% power under the program Activate. This was used to help reduce inflammation and tension and activate the lymphatic drainage system. The laser was then only used at the cervicothoracic region to help blood flow through the brachial artery to the radial nerve. The laser was also placed under the LF armpit and then scanned down the P effected limb caudally.
After the first 5 weeks the patient was seen by Dr. Jeremy. Upon the recheck it showed the P putting down 9% of weight onto the effected. The patient continues to lack active wrist extension, which limits his ability to effectively place the front paw with consistency. Triceps function has improved and the tone in the triceps seems to be improving overall as well, and the patient is able to use passive wrist extension for >75% of steps when walking with excessive shoulder extension to allow for placement of the paw. The patient's ability to consistently see nerve regrowth is vital to long term function in the home and day-to-day life at this time.
